Location/Program Description: Transition 15 is the St. Francis Area Schools transition program for adults ages 18-21 that require additional education past 12th grade in the areas of independent living, post secondary education, employment, community participation, and recreation and leisure. Transition 15 is located in the St. Francis Learning Center and has its own wing of the building with access to public locations of the building such as the gym and cafeteria. The program serves between 15-20 adults per school year with two full time licensed teaching staff and 8 educational assistants.
Position Description: The person hired for this position will have a dual role as a case manager and work experience coordinator. The student’s assigned to your case load will be students that have work experience goals and objectives. As a case manager, you will provide direct instruction, create schedules and programming, coordinate with related service providers, monitor progress, lead team meetings, and complete due process paperwork. You will case manager between 6 and 10 students at a time. In your role as work experience coordinator, you will contact employers and develop community-based opportunities for students to job shadow, have short-term exploratory job placements and longer-term work experiences. Additionally, you will meet regularly with job site supervisors to insure that placement is appropriate for all involved and problem solve as needed, including providing additional training and support to students.
Qualifications: Valid Minnesota Teaching License in work experience. We are looking for a team-orientated individual that enjoys working with adult learners both in the classroom and in the community. We are a fun, hard working team! The ideal candidate would be a creative thinker that can find work experiences in many fields, even with the limitations we have as a smaller, rural community.
