Job Title: Check & Connect Mentor 
Hours: 1.0 FTE: 35 Hours/Week;180 Days/Year
Openings: Chisholm Public Schools, Chisholm, MN

Position Details: 
We are in search of a dedicated full-time Student Mentor to provide individualized academic and behavior support to a defined caseload of students. This position is prorated for 172.5 days for the 2024-2025 school year, and will fulfill 180 days for the 2025-2026 school year. Mentors will work collaboratively with schools and programs to define and outline roles to improve participation, performance, and success for individuals who are disengaging from learning.The interventions include, but are not limited to, small group sessions, one-on-one goal setting, and problem solving. Mentors will collaborate with students, families, school personnel, and community agencies to build relationships and implement a strong support system for students and families. 

Responsibilities:
• Track regularly on target students’ attendance, grades, and behavior, provide regular feedback to students, communicate regularly with families, and collaborate with teachers and other staff members on student progress.
• Analyze data from systematic monitoring to design personalized interventions and document progress.
• Work as a liaison to facilitate communication and promote problem-solving.
• Provide students and families with resources and information to create a link to agencies.
• Attend varied school and district meetings to define and outline roles to improve participation, performance, and success for individual students.
• Performs other duties as assigned.

Other Qualifications: 
• Bachelor’s degree is preferred, in a related field from an approved accredited college or university required. (Additional years of experience may be substituted for the Bachelors Degree)
• Experience in a school setting or similar professional environment preferred.
• Ability to pass a pre-employment background check
